Your dedicated registered dietitian can provide assessments and walk you through a personalized therapeutic nutrition plan that addresses nutrient imbalances and/or deficiencies. This team member can guide you through all things food, hydration, supplements, and more. 

 


 

Blood Glucose Checks 

Blood glucose checks are your power move! Knowing your numbers puts you in control, helping you make smart choices about nutrition, exercise, and medication, and preventing complications like heart disease and nerve damage. Regular checks keep your treatment plan on point, ensuring your blood sugar stays in a safe range. If you’re ever unsure about how or when to check, your HealthCoach is here to help!  

 

Crafting Your HealthProgram Together 

We’re here to build your personalized HealthProgram, tailored to bring positive changes into your daily life. With us by your side every step of the way, you’ll receive customized support, encouragement, and insightful education. By truly listening to you, we’ll help you zero in on key areas like healthy eating, body movement, stress management, and quality sleep. You’ll choose a primary goal, and together, we’ll pick two complementary HealthActivities—your “2x2s”—to practice twice a week. During our monthly check-ins, we’ll celebrate your successes, revisit your goals and activities, and tackle any challenges that come your way. 

 

Your Monthly Check-Ins 

Each month, you’ll have the chance to connect with your HealthCoach and a member of our Clinical Team (either your dietitian or nurse). These one-on-one live calls are all about you—focusing on your health habits, overcoming any obstacles, sharing valuable insights, celebrating your successes, answering your questions, and so much more!
