HIPAA Privacy Notice

Introduction 

 

LIVE TULA has been contracted by your insurance plan, employer, or healthcare provider to provide equipment and services that may help reduce the risk of, or manage well-being with, diabetes and/or other chronic illnesses. Supportive personnel at LIVE TULA will provide tools, education, resources, and encouragement to follow the treatment plan already established by your healthcare provider. 

 

Before using the equipment and supplies in this packet, please review the instruction manuals for each item. All hardware and supplies provided to you by LIVE TULA are for single patient use only and are yours to keep. LIVE TULA does not offer devices for retail sale or rental. Should you need a replacement device, please call 1-855-352-7884. 

 

Notices 

 

The services provided to you by LIVE TULA adhere to the requirements established by the Health Insurance Portability and Accountability Act (HIPAA). HIPAA Rules stipulate that you have certain rights related to the use of your protected health information and that you have the right to be notified of these rights. An overview of your rights and responsibilities, as well as LIVE TULA’s rights and responsibilities, can be found at www.mytula.com/hipaa-privacy-notice. As an Accredited Medicare DMEPOS supplier, LIVE TULA must meet certain standards. Emergency Preparedness 

 

In the event of an emergency or natural disaster that could limit communication with or distribution of supplies from LIVE TULA, we recommend the following precautions: 

  • Keep extra batteries on hand for the operation of the glucometer (see the provided user manual for the type and quantity of batteries).  
  • Maintain at least 30 days of supplies (test strips, lancets) on hand. 
  • Consult with your healthcare provider about how to prepare regarding other aspects of your treatment plan, such as prescription medication, wound care, etc. 
  • During an emergency, if the glucometer becomes non-functional for any reason, or if supplies run out and cannot be obtained from LIVE TULA, any FDA-approved, over-the-counter, self-monitoring blood glucose system may be used (available at local pharmacies) for ongoing management.

If you feel any of your privacy rights have been violated, you may submit complaints to LIVE TULA by calling 1-855-352-7884 or emailing support@livetula.com. Complaints may also be submitted directly to the U.S. Department of Health and Human Services Office for Civil Rights by email at OCRMail@hhs.gov or by phone at 1-800-368-1019. You may also contact our accreditation provider if needed. Our accreditation provider is HQAA and can be reached at 866-909-4722. The law forbids us from taking retaliatory action against you if you submit a complaint.
